Service Structure for Medication Reminders

Medication Reminders are a non-medical support service where caregivers or personal support workers assist clients in remembering to take their prescribed medications on time and in the correct dosage as directed by their healthcare provider. This service does not involve administering or altering medication (unless the caregiver is a licensed nurse), but rather ensures adherence to the client’s prescribed routine for better health outcomes.

Assessment & Planning

  • Review the client’s prescribed medications, timing, and frequency.
  • Create a customized medication schedule (daily/weekly).
  • Set up pill organizers, blister packs, or reminder tools if needed.

Reminder Delivery

  • Caregiver provides verbal or visual reminders at the scheduled time.
  • May include phone calls, in-person prompts, or electronic reminders.
  • Ensure the client acknowledges and takes the medication independently.

Support Activities

  • Reading medication labels for clients with vision difficulties.
  • Offering water or assistance in opening bottles/blister packs.
  • Documenting that the reminder was given and medication was taken.

Monitoring & Reporting

  • Observe for missed doses and report patterns to family/health professionals.
  • Notify family or nurse if the client refuses or forgets medication repeatedly.
  • Track side effects or unusual reactions and escalate appropriately.

Boundaries of Service

  • PSWs/caregivers provide reminders only; they do not administer injections, alter dosages, or make medical decisions (those require licensed nurses or doctors).
